Affidavit for Change of Name

IN THE MATTER OF AFFIDAVIT FOR CHANGE OF NAME

AFFIDAVIT

I, deponent_name, son/daughter of deponent_father, resident of deponent_address, holding CNIC No. deponent_cnic, solemnly affirm and declare as follows:

1. That I am a law-abiding citizen of Pakistan and am competent to make this solemn affirmation in support of my request for formal recognition of my changed name.

2. That I was originally known by the name original_name, as evidenced by my original birth certificate / CNIC bearing the said name.

3. That due to reason_for_change, I have now decided to change my name from original_name to new_name.

4. That I have been consistently known and called by the name new_name since date_of_change in all matters, official documents, and dealings.

5. That I have not changed my name for any unlawful purpose, nor to evade any legal liability or obligation.

6. That I am willing to use the name new_name in all future legal, official, and personal transactions.

7. That the supporting documents confirming the consistency of my use of the new name are attached hereto as Annexure 'A', which includes educational certificates, employment records, and other official correspondence in the name new_name.

8. That there is no objection from any party, and no legal dispute exists regarding the use of my new name new_name.

9. That I make this solemn affirmation in my true and correct capacity and with complete knowledge of the consequences of making a false affirmation.
