IN THE MATTER OF AFFIDAVIT FOR DATE OF BIRTH CORRECTION
Note: Stamp Paper of Rs. stamp_paper_value affixed hereto.
AFFIDAVIT
I, deponent_name, son/daughter of deponent_father, resident of deponent_address, holding CNIC No. deponent_cnic, solemnly affirm and declare as follows:
1. That I am a law-abiding citizen of Pakistan and am competent to make this solemn affirmation in support of the correction of my date of birth in official records.
2. That my correct date of birth is correct_dob, as evidenced by my birth certificate issued by issuing_authority on certificate_issue_date.
3. That there is an error / discrepancy in my official records, including my CNIC and other documents, which currently show my date of birth as incorrect_dob.
4. That this discrepancy has caused me considerable difficulties in obtaining visas, employment, educational admissions, and in various official matters.
5. That the error occurred due to cause_of_error.
6. That the correct date of birth correct_dob is supported by the following authentic documents:

7. That I have not committed any fraud or misrepresentation, and this correction is sought to rectify a genuine clerical or administrative error.
8. That all the information contained in this affidavit is true and correct to the best of my knowledge, and I understand the legal implications of making a false affirmation.
