Subject: Application for grant of membership in society_name, registered under the Cooperative Societies Act 1925 / the relevant provincial Cooperative Societies Act, in compliance with the bye-laws of the society.

Respected Sir / Madam,

1. That the undersigned, applicant_name, son / daughter of applicant_father, holder of CNIC No. applicant_cnic, resident of applicant_address, occupation applicant_occupation, hereby applies for membership in society_name for the purposes envisaged in the bye-laws of the society.

2. That the undersigned is eligible for membership of the society and qualifies under the relevant clauses of the bye-laws, namely eligibility_clauses, in particular, the requirements as to age, residence, profession, and capacity, the particulars of which are submitted with this application.

3. That the undersigned undertakes to: (a) abide by the bye-laws, rules, and regulations of society_name, as well as the directions and resolutions of the Managing Committee from time to time; (b) make timely payment of subscriptions, contributions, and any liability that may be assessed against the undersigned in accordance with the bye-laws; (c) participate in the affairs of the society in accordance with the principles of cooperation; and (d) inform the society promptly of any change in the contact details, profession, or particulars provided in this application.

5. That the following documents and particulars are submitted in support of the application: (a) attested copies of CNIC of the applicant and the proposing member (if required); (b) attested copies of the proof of profession / income; (c) two recent passport-size photographs of the applicant; (d) proof of payment of admission fees and share-money; (e) declaration of eligibility (annexed); (f) additional_documents; and (g) any other document required under the bye-laws.

It is, therefore, respectfully prayed that society_name may be pleased to: (a) consider the present application along with the supporting documents on priority; (b) admit the undersigned as a member of the society, allotting the prescribed number of shares and member identity; (c) communicate the membership number, share certificate, and identity card of the undersigned in due course; and (d) issue any further intimation as may be required in the matter.
