Respectfully submitted:

1. I / We, taxpayer_name, NTN taxpayer_ntn, hereby apply for inclusion in the Active Taxpayers List (ATL) under Section 182A of the Income Tax Ordinance 2001.

2. I / We acknowledge that we are a late-filer and that the tax return for the tax year applicable_tax_year was not filed within the prescribed due date of original_due_date.

3. The tax return was filed on actual_filing_date, which is days_late days after the due date.

4. In accordance with Section 182A of the Income Tax Ordinance 2001, I / We have paid the surcharge on the late filing of the tax return.

(b) Grant such other relief as may be deemed just and proper in the circumstances.
